Obtaining a release shows a level of care for your client and their families as well as protecting yourself should they decide to no longer work with you. This release is meant for midwives, doulas, birth photographers and more to use, we want it to be as flexible as your business needs it to be. The release as it is given is a long one, it has a lot of parts that you may or may not need for your particular business. That’s fine, feel free to edit it as needed. The only part that is not removable is the copyright at the bottom, that must be left intact as it denotes who created the form originally.
